Find Work as an Individual Provider
Contact the Referral Registry Center in your area. You can visit or contact one of our statewide locations, by telephone or e-mail. The staff are available to help you learn more about becoming an Individual Provider and how enroll and find work using the Referral Registry.
The Referral Registry connects Individual Providers with those who need In-Home services. Applicants must be at least 18 years old, successfully complete a background check, a face-to-face interview and an introductory course before being included on a provider list.
